Casera Raises $1M to Bring AI Automation to Hospital Case Management

Seattle healthcare startup Casera has officially emerged from Pioneer Square Labs (PSL) with a mission to improve hospital operations by using agentic artificial intelligence to automate case management and reduce delays in patient care.
The company has secured $1 million in funding from PSL as it develops technology designed to eliminate operational bottlenecks that often extend hospital stays and increase healthcare costs.
Rather than focusing on analytics dashboards, Casera is building AI-powered software that actively helps hospital teams complete critical tasks, enabling patients to move through the healthcare system more efficiently.
Using Agentic AI to Reduce Hospital Delays
Hospitals frequently face operational challenges that keep patients admitted longer than necessary. Delays involving communication between care teams, insurance authorization, discharge planning, and task coordination can add days to a patient’s stay while significantly increasing costs.
Casera aims to solve these issues with what it calls a “Case Manager Digital Agent.”
The AI-powered assistant works alongside hospital case managers, professionals responsible for coordinating the many operational steps required before patients can safely leave the hospital or move to the next stage of care.
Instead of simply identifying problems, the platform monitors communication channels, understands the context of ongoing cases, and automatically triggers the next appropriate action.
For example, the software can follow up on pending insurance prior authorizations, assign ownership for discharge-related tasks, create deadlines, and ensure important steps aren’t overlooked.
Built to Work With Existing Hospital Systems
According to CEO Neeraj Singh Bhavani, Casera’s platform integrates directly into hospitals’ existing collaboration and communication tools instead of requiring organizations to replace their current workflows.
The software helps determine what actions need to happen, identifies the people responsible, and keeps processes moving until every required task is completed.
Bhavani previously founded patient-flow technology company Tagnos, which was later acquired by Sonitor.
He believes Casera approaches hospital operations differently than traditional patient flow vendors.
A Different Approach From Existing Competitors
Several established healthcare technology companies already provide hospital capacity management and patient-flow solutions, including Qventus, LeanTaaS, and TeleTracking.
However, Bhavani says Casera focuses on execution rather than simply providing recommendations.
“We’re not trying to be another legacy dashboard and analytics player,” he said.
Instead of telling hospitals what should happen, the platform is designed to help ensure those actions actually get completed through intelligent automation.
Experienced Healthcare Leadership
Casera’s leadership team combines expertise in healthcare operations, software development, and clinical workflows.
Alongside Bhavani, the startup was co-founded by CTO Alex Levin, who previously launched revenue intelligence company MD Clarity, which was later acquired by a private equity firm.
Another key early leader is Jhayne Pana, who previously served as an assistant nurse manager at MultiCare Health, bringing firsthand experience with hospital operations and patient care.
The startup is currently working with design partners across major health systems in three different states to refine its platform before broader commercialization.
Although the company has not yet generated revenue, it is already testing its technology with healthcare organizations.
Backed by Pioneer Square Labs
Casera currently employs fewer than ten people and represents another healthcare-focused startup launched through Pioneer Square Labs, Seattle’s startup studio.
PSL has previously spun out successful healthcare companies, including staffing software startup Kevala, which was acquired earlier this year.
T.A. McCann, Managing Director at Pioneer Square Labs, believes hospital workflow automation presents one of the strongest opportunities for agentic AI.
He noted that improving patient flow through automation addresses a significant market need while highlighting the value of working with experienced founders who have already built and exited successful healthcare companies.
With early funding secured, experienced leadership, and growing interest from hospital partners, Casera is positioning itself to modernize hospital case management by replacing manual coordination with AI-powered automation. If successful, the platform could help healthcare providers reduce unnecessary hospital stays, improve operational efficiency, and ultimately deliver better experiences for both care teams and patients.
